SPH Home Health is a network of programs designed to meet the needs of elderly and disabled homebound individuals choosing to remain at home.

South Peninsula Hospital Home Health is a network of programs designed to meet the needs of elderly and disabled homebound individuals choosing to remain at home in their community. We work closely with each client, their families, physicians, and other agencies to provide an individualized care plan for people of all ages.

South Peninsula Hospital Home Health Department was named a Top Agency in the 2018 HomeCare Elite recognition list of top-performing home health agencies in the United States. SPH Home Health was listed in the top 25 percent of the nearly 9,000 agencies considered.

HomeCare Elite agencies are determined by an analysis of performance measures in quality of care, quality improvement, best practices implementation, patient experience (HHCAHPS), and financial management. In order to be considered, an agency must be Medicare-certified and have data for at least three outcomes in Home Health Compare. Out of 8,898 agencies considered, SPH was one of the 2,223 recognized in the top 25%, and only one of two named in the state of Alaska.

HomeCare Elite has annually identified the top 25 percent of Medicare-certified agencies for 13 years. The ranking is developed by ABILITY® Network, a leading information technology company helping providers and payers simplify the administrative and clinical complexities of healthcare. Home Health – We provide intermittent skilled services as prescribed by their physicians including:

  • Skilled Nursing care
  • Physical Therapy
  • Speech Therapy
  • Occupational Therapy

Our goal is to help clients become independent in their homes by providing:

  • Disease process education/teaching
  • Wound care
  • Ostomy care
  • Observation and assessment
  • Medication management

Lifeline – With Lifeline, your loved ones can rest assured that you’re safe, secure and happy in your home. Please arrive 15 minutes prior to your scheduled appointment. Have your photo ID, insurance card, and any relevant medical records ready for our front desk team.

You will meet with your provider for an in-depth consultation. We take the time to listen, answer your questions, and formulate a customized treatment plan tailored specifically to you.

All of your test results, visit summaries, and medical records are securely available 24/7 through our online Patient Portal. You can easily sign up or log in from the main navigation menu.
Do I need a referral from my primary care provider?
This depends on your specific insurance plan and the type of specialty care you are seeking. We recommend checking with your insurance provider or calling our front desk team to determine if a referral is necessary.
